Pushing and B I RTH – Second Stage

Mother’s Feelings And Reactions

• Finds strength and energy; “light at the end of the tunnel” feeling.

• Finds it difficult to push in the beginning; “on the job training” since there was no practice in class. • May find pushing as relief; distracts mother from discomfort of contraction. • Occasionally, belching and passing gas. • May continue with the shakes.

• Burning and stretching sensation as baby moves down the birth canal. • The hardest physical work you will ever do.

Physical Changes In Mother

• May have contractions less frequently; may space out to be every 3 to 5 minutes, lasting 60 to 90 seconds.

• Has a strong, natural urge to bear down with the contractions (epidural eliminates this sensation). • May see bulging of the perineum and rectum along with a bloody vaginal show. • May fall asleep between contractions.

• May make noises or grunt with contractions due to the enormous amount of pressure felt.

Role Of The Support Person

• Encourage her every step of the way.

• Help her to push properly by positioning her correctly. • Encourage her to find her own best position for pushing. • Offer cool washcloths and ice chips.

• May need to pant/blow with her as the head crowns when healthcare provider requests that she stop pushing.

• Have staff help you with “crowd control” if it becomes a problem. • Focus your attention on the birth of your baby.

• May be screaming with delight or may be overwhelmed and want to sleep. • Involved with how baby is doing; asking questions about baby’s well-being.

Physical Changes In Mother

• Slowing of contractions after birth of baby.

• Shrinking of uterus to grapefruit size; found at level of the umbilicus.
