Act i ve Labor – F i rst Stage

Mother’s Feelings And Reactions

• No longer social; very much involved in the work of labor.

• More serious and less talkative; concentrates and focuses on techniques that work for her. • Selectively attentive.

• Unable to understand conversation.

• Desires and needs companionship from support person.

Physical Changes In Mother

• Contractions progressively stronger and closer together; 3 to 4 minutes apart and lasting 40 to 60 seconds.

• Longer peaks in contractions, and more uncomfortable as time moves on. • Pressure or tightening in pubic area. • May experience dry mouth and perspire.

• Appears annoyed, more restless, unable to relax, may hyperventilate. • May experience nausea and vomiting. • May appear pale or flushed.

Comfort Measures Mother Can Utilize

• Do not fight the contractions; you must allow the contractions to do their job. • Continue with breathing and relaxation techniques that work for you. • Incorporate abdominal massage with breathing techniques.

• Use focal point with contractions. This will help you to externalize and not think and focus on the discomfort of labor.

• When the contraction is over, do not think about the next one coming; that is wasted energy. Close your eyes and relax into the bed.

Role Of The Support Person

• Give her your undivided attention, especially during a contraction. You may have to breathe right along with her to keep her focused.

• Re-evaluate coping strategies and breathing techniques whenever mother seems discouraged or feels like she can’t go on.

• Make sure commands are short and to the point.

• Verbal support and encouragement are now extremely important. • Anticipate physical needs, which are many: – Keep her cool with a wet washcloth kept on ice. – Remind her to empty her bladder every hour. – Keep her lips moist with a water-soluble substance. – Offer ice chips.

– Surround her with pillows to keep her comfortable and relaxed. – Apply pressure to lower back if needed.

– Apply touch, massage, and relaxation techniques. – Help her change positions frequently.
